Participant Complaint Form
Participant complaints or concerns must be submitted in the form provide on this page.
We will attempt to resolve minor concerns or complaints with the complainant informally, often referring the participant back to the Committee Chairman. If the concern or complaint is significant, it shall be investigated in a more formal manor with the OISF Board of Directors.
